I too have this issue and can confirm it’s when the game crashes with the buzzing sound at loading screens. I reckon that the issue stems from the number of crashes that were happening to me at the end of some combats after playing for an extended time (something along the lines of it has updated the local save file, but crashed before it updated the Xbox achievement tracker). The Dealing With Thieves is painful after a reset as I need to play the majority of the game again to try to unlock that as I have to complete The Devil again, so haven't verified that one just yet.
